Sharing workshop on international experience in the development of family physician model in providing services to strengthen primary health care (12/01/2016)
The primary objective of the health system in which each country including Vietnam is aiming for a sustainable development through the implementation of health care for the entire population, that is ensuring that all citizens need can be use of advanced health services, prevention, treatment, rehabilitation and palliative care quality and efficiency while ensuring that the use of these services does not make users encounter to financial risks. To accomplish this goal, the decisive factor is the health care system should be based on the foundation of primary health care through the strengthening of grassroots health care network.
In our country, over many years; The Party and Government always identified health facility is the basic foundation of the health system. Therefore, The Party and the Government has issued many important decisions as to constantly consolidate, improve and enhance the operational efficiency of the health care facility. Therefore, Vietnam has gained many achievements in the health indicators of the population especially with the millennium objectives in the field of health care.
However faced with the challenge of change on disease patterns, population aging problem with the increase in NCDs with dangerous diseases emerging health network requires establishments to be must continue to innovate to meet not only with the infectious disease prevention and health care usual but also to meet the needs of health care for non-communicable disease in which the patient does not only receive health care at the health facility, but in the family, community and health management. The health sector in addition to providing care services and also to guide treatment for patients, relatives and family members in the community to improve their health, disease prevention and rehabilitation. Health care also requires participation in close coordination of individual, family and community.
International experience shows that family medicine has an important role in helping to strengthen the operational capacity of the health system, contributing to the effective work of primary health care under the motto care National Health Care. Family Medicine helps people reduce the cost of health care, health promotion through early access to appropriate health services, which are comprehensive care and continuity and reduce inequalities in care health. Activity patterns of family physician (GP) to ensure that all people get health care in the community, giving more emphasis to preventive health promotion, disease screening, health care committee head and health management with the active participation of individuals, families and communities. Many countries around the world always appreciated and honored role of family doctor and consider them as the "gatekeepers" of the system of medical service providers. When sick, the first thing is that the patient must have access to family doctors for treatment and if necessary will be introduced family doctor referral. If patients present themselves to health facilities will have to pay for health care with very expensive price. The organizational model of family medicine activities in the world are quite diverse and are applied flexibly depending on the terms of natural, economic, social and disease patterns of each country. Brazil has 40,000 family medicine team with ingredients include: family doctors or interns, 2 nurses and 6 medical collaborators. Many other countries, including Turkey, Cuba, Australia, the US, China has models GP Clinics independent or integrated into public health facilities, non-public and operate very efficiently ... The country also has many successful experience in personnel training in the field of family medicine and the amount spent on training in the community. Evidence from many countries around the world also showed the practical benefits from family medicine activities for the health care of the people. Family Medicine can solve 70% of health care needs of the people first. There are countries that although economic development has not yet implemented the model by family physicians that population health condition improved quickly.
Before that scene, on 12/01/2016, under the direction of leaders of the Ministry of Health, the Health Strategy and Policy Institute held a Sharing workshop on international experience in the development of family physician model in providing services to strengthen primary health care in Hanoi, with the support of the Atlantic Philanthropies. The workshop is run by Prof. Ph.D. Pham Le Tuan - Deputy Minister of Health, Prof. Ph.D. Luong Ngoc Khue - Managing Director Health, and the special guest of the seminar, Prof. Michael Kidd – chairman of Global Family Doctor (WONCA). Seminar with the participation of Mr. Nguyen Minh Thao - Deputy General Director of the Vietnam Social Insurance, leaders and experts from departments under the Ministry of Health as Clinical Management Department, Department of Preventive Medicine Department of Science, Technology and Education, Department of Planning - Finance, International Cooperation Department; Leaders and researchers of the Health Strategy and Policy Institute; representatives of several universities across the country, including the University of Medicine in Hanoi, Ho Chi Minh City Medicine and Pharmacy University, Hue University of Medicine, University of Public Health; representatives of health facilities at all levels, such as the Central Hospital of Gerontology, Medical Center of Soc Son; and representatives of local and international organizations, such as the World Bank (WB), the Center for Training and Community Development (RTCCD) ...
Through the experiences and opinions are exchanged in the workshop, Prof. Ph.D. Pham Le Tuan - Deputy Minister of Health concluded family physician model (GP) can be adapted flexibly and development in the context of Vietnam and to consider it as one of the useful solutions help enhance supply and access to health care services by the original family physicians have an important role in health care towards disease prevention, health care continuous, comprehensive, family oriented and community. Regarding the mode of operation may apply flexible model both GP clinics in urban areas, rural areas and the pattern plain family medicine team to integrate the operations of CHS in the area rural areas, remote and difficult ... On human resource training for family medicine field will be deployed at the base of medical education in the country and more focused training form communities associated with the practice at the health unit basis. Also need to enact coordination mechanisms linking transit between family doctor clinics, family medicine team with public health facilities as well as non-public. Besides need to diversify investment resources for family medicine activities depending manner of operation, may from investments of state or through health insurance or reimbursement ... The development model GP clinics and family medicine team to integrate the operations of CHS will contribute to addressing overcrowding in Vietnam hospital today and ensure that people are able to access to health services at online quality medical facilities as well as reduced health costs for families and society....
